Yeah, the form is for a lottery to win a prize, so filling it out is optional (and probably limited to those living in Japan). To vote, just press the red button with "投票する" at the bottom of the page.

 

She won the 2nd place on the gravure competition! She had a total of 65,981 points while the winner, Amaki Jun (From Kamen Joshi), had 66,005, so it was pretty close.

 

This is pretty interesting, some kind of survey or field show hosted by Black Mayonnaise gathers workers from the same field and asks their opinions or for interesting details they've encountered in their profession i believe. Anyway this ep was on gravure Idols and Shiori was one of the idols invited, but one of the interesting details is that Sato Seira was also one of the gravure Idols invited, and while there isn't too much there is some back and forth between the two. (No mention of SKE though).

I believe she was featured in the 50th anniversary of Weekly Playboy this week

 

Shiori is dressing up as idols from 1970s to modern day. It is in the 50th anniversary issue of Weekly Playboy
